Robust Management of Motion Uncertainty in Intensity-Modulated Radiation Therapy

چکیده
Radiation therapy is subject to uncertainties that need to be accounted for when determining a suitable treatment plan for a cancer patient. For lung and liver tumors, the presence of breathing motion during treatment is a challenge to the effective and reliable delivery of the radiation. In this paper, we build a model of motion uncertainty using probability density functions that describe breathing motion, and provide a robust formulation of the problem of optimizing intensity-modulated radiation therapy. We populate our model with real patient data and measure the robustness of the resulting solutions on a clinical lung example. Our robust framework generalizes current mathematical programming formulations that account for motion, and gives insight into the trade-off between sparing the healthy tissues and ensuring that the tumor receives sufficient dose. For comparison, we also compute solutions to a nominal (no uncertainty) and margin (worstcase) formulation. Unlike the nominal solution, which delivered an unacceptable 6% average underdose to the tumor in our 78 test cases, the robust solution resulted in only a 1% worst-case underdose. In addition, the robust solution reduced the total dose delivered to the main organ-at-risk (the left lung) by roughly 11% on average, as compared to the margin solution.

متن کاملThe evaluation of lung doses for radiation pneumonia risk in stereotactic body radiotherapy: A comparison of intensity modulated radiotherapy, intensity modulated arc therapy, cyberknife and helical tomotherapy
Background: Radiation Pneumonia (RP) is one of the most extensive side effects in Stereotactic Body Radiotherapy (SBRT) of lung cancer. SBRT are performed by means of Intensity Modulated Radiotherapy (IMRT), Intensity Modulated Arc Therapy (IMAT), CyberKnife (CK) or Helical Tomotherapy (HT) treatment methods. In this study, we performed a plan study to determine the plan parameter such as the M...

Introduction: Intensity modulated radiation therapy (IMRT) is one of the cancer treatment methods. It is important to selectively aim at the target in this way, which can be performed using a multileaf collimator (MLC). Materials and Methods: In order to specifically irradiate the target volume in radiotherapy to reduce the patient absorbed dose, the use of multileaf collimator has been investi...

متن کاملThe Quality Control of Intensity Modulated Radiation Therapy (IMRT) for ONCOR Siemens Linear Accelerators Using Film Dosimetry
Introduction Intensity Modulated Radiation Therapy (IMRT) has made a significant progress in radiation therapy centers in recent years. In this method, each radiation beam is divided into many subfields that create a field with a modulated intensity. Considering the complexity of this method, the quality control for IMRT is a topic of interest for researchers. This article is about the various ...
